You must be able to carry out the following duties...

DUTIES & RESPONSIBILITIES

1. Work with the person and other professionals, as necessary, in order to facilitate the stated functions of the first response service. This includes information gathering, the undertaking of proportionate assessments, access to fast track support, signposting and the provision of information and advice, promoting choice and control wherever possible.

2. Undertake trusted assessment/provision functions to promote independence and, where appropriate, offer access to standard equipment, which may involve the issuing of an equipment prescription (for redemption via a retail outlet) and delivery/fitting. Staff can be asked to undertake face to face interviews at their place of work and home visits, but home visits would not be routinely expected.

3. Identify and process any safeguarding and quality of care issues and refer on to the appropriate staff/agency to ensure that the persons' welfare is protected.

5. Develop and maintain effective working relationships with the Access to Resources team, private and third sector brokers and the Shorter Term and Longer Term support services to ensure the smooth transition of people through available pathways.

6. Initiate and develop close joint working with health related agencies (including GP's and Intermediate Care), the third sector, local housing providers and other agencies, on behalf of people and the authority.

7. To promote and empower people and their families and carers, to ensure they receive opportunities to access broader reablement support with a view to regaining or increasing levels of independence, as far as possible.

8. To communicate effectively and sensitively with people and carers, regardless of their spoken language or communication status. Adapt communication techniques as the situation demands making best use of Directorate informational resources and other methods of communication.

9. Maintain the Directorate's records and other data accordingly ensuring that it meets the relevant and stated levels of accuracy, quality and timeliness required and complies with statutory requirements, whilst maintaining confidentiality.

10. Ensure value for money principles are applied in all areas of service delivery in consultation with supervisor, manager and partner agencies

11. In order to deliver flexibility of staff movement and facilitate optimal service delivery, the postholder will be able to, and may be required to, transfer to other equal Officer graded posts within AHWB operational services area.
