An exciting new opportunity has arisen for a Placement Officer to join a local organisation based in Essex.

The successful candidate will be responsible for the following duties:

·  To commission approved packages of care with Reablement providers and homecare providers and ensure timely, safe and smooth transfer of those clients in the care of those providers.

·  To commission Respite and Long Term Residential/Nursing Home Placements for clients and ensure timely, safe and smooth transfer of those clients to their chosen Home.

·  To ensure that the requirements of the Dynamic Purchasing system, as introduced within ASC, are understood and that all practice and procedures are compatible with the needs of the system

·  To meet the requirements of increasing Direct payments and Individual service funds and advise service users and providers alike of all information necessary to facilitate a smooth process to self-directed support where applicable
